complete the tuition assistance request form
Complete the tuition assistance request form and secure the necessary approval before you enroll in the course(s). Send the completed form to the Training and Development Department at MS107.
provide documentation to receive reimbursement
After you have completed the course(s) and received your final grade(s), you must provide the Training and Development Department with the following documentation in order to receive your reimbursement:
- An itemized, zero balance receipt tuition and fee receipt.
The Bursar’s Office in the university will provide you with this receipt. The itemized receipt must indicate that expenses were for tuition and fees. The Laboratory requires a description of the specific charges so that disallowed expenses are not included. A receipt that indicates “Balance Forward” or “Paid on Account” is not acceptable since it does not describe the specific charges. The receipt must also show a "zero balance" to ensure that no credit is due the Laboratory (see examples).- A receipt for each required book purchased.
- An official copy of your final grade(s).
failure to provide full documentation within 60 days
All documentation is due upon course completion. If any required documentation is outstanding 60 days after course completion, you will forfeit your reimbursement.
